Humana Claims Payment Policies

 Effective July 1, 2026, Humana will charge a $5.00 administrative fee for Medicare Advantage paper claim submissions.  

The fee will apply only when the reimbursement amount for the claim is $10.00 or more. This policy is being implemented as part of Humana’s effort to reduce administrative costs and eliminate the risk of lost mail. Humana previously implemented a $5.00 per envelope charge for paper checks when the amount is $5.00 or more.
